Memory Care Assisted Living

Private or shared rooms range in price from $4,200 to $4,450

In both cases prices are all inclusive of:

Room and Utilities (except for phone, internet and cable)
Three meals each day
Snacks
Assitance with activities of daily care
Personal and linen laundry
Self Assisted Medication Management (SAMM)
Housekeeping
Activities

One Time, Non-refundable Community/Admission Fee $2,000

Assisted Living Subsidies

Medicaid Subsidy Available for both programs: Group Adult Foster Care (GAFC) which is a medicaid subsidy for assisted living.

Eligibility: (1) (requirements subject to change) Assets less than $2,000, monthly income less than $1,111, medical diaganosis, need help with at least one activity of daily living (ADL) (2)Assets less than $2,000, monthly income greater than $1,111, with a six month spend down and medical diaganosis, need help with at least one activity of daily living (ADL)

Benefits: (subject to change) Fee of approximately $1,150 paid directly to the residence, individual's monthly income supplemented to a total of $1097..

Veteran's Subsidy of up to $1,519 per month is also available for the veteran and a subsidy of $976 is available for the surviving spouse.
